This is a Refugee Health Case Manager position. The case manager is primarily responsible for monitoring the communication among healthcare providers, refugee resettlement agencies, and the county health department (CHD) to ensure refugees avail themselves to follow-up care. The Refugee Health Case Manager will ensure that clients in need of follow-up care are linked to the appropriate medical care provider for ongoing medical care and treatment. The incumbent will also be responsible for working with local service providers including refugee resettlement agencies, CHD staff, and other refugee service providers to facilitate reciprocal client referrals.

Emergency Duty Responsibilities - In the event of emergency (any threat of disaster or disaster), employees may be required to perform emergency operations functions. Must be willing to work during/beyond normal work hours/days in the event of a disaster.

Position requires a current certification in CPR. New employee must be willing to obtain CPR certification within the first 6 months of hire. Training is provided through the Hillsborough County Health Department.

The primary duties of this position are in case management Refugee Programs. The incumbent has good rapport with staff and clients and adheres to established Department of Health policies and procedures; evidenced by professionalism, approachability, a positive attitude, and flexibility.
Monitoring the communication among healthcare providers, refugee resettlement agencies, and the county health department (CHD) to ensure refugees avail themselves to follow-up care. The RHCM will ensure that providers report back to the CHD regarding the follow up care the client received.
Working with local service providers including voluntary agencies, CHD staff, and other refugee service providers to facilitate reciprocal client referrals. The RHCM will provide the following assistance to refugees referred to them:
a. Assist with transportation to medical appointments for follow-up healthcare.
b. Serve as a guide through the reapplication process for Medicaid, or other health benefits, after the initial eight months of eligibility.
c. Provide needed assistance with healthcare and mentor the refugee until he or she is ready to access healthcare independently.
d. Ensure completion of treatment for latent tuberculosis infection (LTBI).
Providing training to refugee resettlement agencies, in such areas as, but not limited to, communicable diseases, how to work with clients with communicable diseases, and how agency staff can protect their personal health. The RHCM may accomplish this task by providing training to community partners on such issues as working in close contact with a refugee identified as having LTBI.
Serving as a health care resource by working with refugee resettlement agencies’ staff during the first three months of resettlement to ensure that all refugees receive information about how the healthcare system works. The RHCM should determine the education currently provided by their community partners through task force meetings or
other regularly scheduled meetings with community providers. The RHCM should identify gaps in the provision of health-related education to refugees and assist in filling those voids.
Developing and maintaining a list of local physicians, by specialty group and location, who have the capacity to provide qualified in-office interpreters for specific languages. The RHCM should work with the local VOLAGs to obtain current listings of physicians in their community and expand the list as additional service providers are identified.
Participating in local task force meetings related to refugee services and refugee healthcare. The RHCM will provide valuable feedback to meeting participants about the unique needs of refugees requiring follow-up medical care. The RHCM will establish collaborative referral relationships during these meetings and address any barriers to accessing services for their clients.
