Patient Care Coordinator (Mendota Heights, MN)

Manage all patients in the monthly call cycle and escalate need for contact, if patient is not reached.
Functions as contact person between all members of the healthcare team to relay necessary information in order to coordinate patient needs.
Identifies customer issues that require the attention of a nurse, pharmacist, consumer advocate, or the admissions team.
Documents all communications in the patient clinical chart and obtains co-signatures from a nurse or pharmacist that relate to patient care.
Initiates Discharge Summaries, discharges patient therapies in the system, and changes patient status to discharge for Company nursing patients.
Assists patients in maintaining appropriate inventory levels of all needed products, supplies, and monitors usage.
Identifies potential inventory/delivery problems and initiates actions to resolve them.
Provides assistance with overseeing factor inventory to ensure that inventory levels are appropriate and that factor is available for all scheduled and most emergent shipments. Ensures all other needed supplies are available and added to inventory if necessary.
Assembles and maintain patient clinical charts and files documents. Gathers information for the initial admission packets (Patient Resource Guide) for new patients and reviews all new charts to ensure that all necessary nursing paperwork is included.
Communicates with medical professionals and insurance company personnel to expedite patient eligibility into program.
May resolve reimbursement issues with accounts, sales force, and management.
Completes data entry in CPRS to ensure accuracy of reporting and outcomes.
Assists the admissions team in resolving authorization or other insurance related issues.
Investigates data entry issues to correct data warehouse reports.
Enters initial patient delivery ticket with appropriate billing information.
Monitors computerized reports to avoid delays in customer billing information.
Follows up with PFSC or NCC to assure timely resolution to un-voucherables.
Compile information for reports at the request of the Supervisor.
Performs other related duties as directed by supervisor.
Qualifications:
Education or experience equivalent to a high school diploma or GED; some college preferred.
At least three years related experience in coordinating activities, administrative support functions, customer service, experience as nursing assistant, pharmacy technician, home patient representative, or customer service representative is preferred.
Excellent verbal and written communication, computer, and organizational skills are necessary
At least a moderate level of experience with Excel and the ability to quickly learn other software systems as required for data entry and reporting.
Physical Demands:
The employee is regularly required to talk or hear.
The employee is frequently required to stand, walk, and sit.
The employee is frequently required to use hands and reach with hands and arms.
The employee is frequently required to lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
Vision requirements include close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
WORK ENVIRONMENT:
The employee may be exposed to fumes or airborne particles.
The employee may be occasionally exposed to toxic or caustic chemicals.
The employee occasionally works near moving parts and electrical equipment.
