Program Manager
The role of the Treatment Coordinator is to provide individual substance abuse counseling and case management to Clients of Casa Esperanza, Inc. using a Motivational Counseling approach and providing the 12 core functions: screening, intake, orientation, assessment, treatment planning, counseling, case management, referral, education, crisis intervention, and record keeping. All candidates should be CADAC certified (or a reasonable equivalent), or actively pursuing this certifications. Job Responsibilities: * Coordinates Intake and Admissions to the program * Conducts initial assessment for each client and generates individualized treatment and service plans * Provides the 12 core counseling functions to each assigned client: screening, intake, orientation, assessment, treatment planning, counseling, case management, referral, education, crisis intervention, record keeping * Coordinates internal and external services, i.e. legal, medical, housing, employment, training, etc. * Works closely with clinical team to ensure client individualized treatment and service plans are completed * Coordinates services by referring clients to appropriate outside providers such as: medical, housing, financial, and other services * Collaborates with other agencies, public organizations, and external personnel to develop comprehensive network of services. * Completes clients admissions and discharge forms * Completes psychosocial assessment * Develops a treatment plan with clients to guide sessions and address needs based on Casa Esperanza Clinical Logic Model * Manages medical treatment plan, i.e. appointments, medication, HIV supportive services and referrals * Provides individual client sessions once per week * Documents all client encounters and progress, completes necessary reports, and maintains up-to-date client files * Participates in weekly clinical team meeting to address client needs * Manages crisis interventions as needed * Facilitates weekly Anger Management/Relapse Prevention groups * Conducts urine screening for clients as needed * Develops aftercare plan with the client prior to the end of treatment * Meets bi-weekly with program manager for supervision * Coordinates client transportation as needed * Work some evening and weekend hours * Other duties as required Skills necessary for the position: * Ability to use Motivational Counseling/Motivational Interviewing techniques * Familiarity with Cognitive Behavioral Therapy * Ability to work as a member of a team * Ability to work independently * Demonstrated capacity to make sound clinical decisions and manage crises * Knowledge of substance abuse field, including the AA/NA 12 Step philosophy * Knowledge of appropriate community resources * Knowledge of HIV/AIDS service planning and education * Strong verbal and written communication skills * Crisis management skills * Valid Massachusetts Drivers License preferred * Computer skills, minimum Microsoft Office * Experience with homelessness, mental illness, domestic violence Experience and education necessary for the position: * Minimum of 2 years experience in substance abuse field * Associates/Bachelors Degree preferred (or comparable experience) * CADAC certified (or reasonable equivalent), or actively pursuing this certification. * Bilingual Spanish/English required * CPR certified How to Apply: Please send your cover letter and resume to Jessica Cohen, Operations Director, at [click on "Apply Online" button].
