Founded in 1989, Illinois MENTOR - a partner of The MENTOR Network - has emerged as a leading provider of home and community-based services to adults with intellectual and developmental disabilities (IDD) and children/youth with behavioral, emotional or complex medical needs. Illinois MENTOR is accredited by COA and our services include a range of residential options, in-home supports and our Specialty Foster Care and Host-Home programs. We currently are seeking an ambitious and energetic Program Services Coordinator to be a part of our team in the Matteson/Schaumburg area. Position Summary: The Program Services Coordinator coordinates development and implementation of services/treatments provided to clients/consumers; oversees all services provided to individuals and their families; monitors quality of care delivered; ensure Mentors comply with contractual terms; and monitors corporate and regulatory program compliance. Key Responsibilities:
Assesses referred clients and coordinates the development of a service plan in conjuction with a treatment team and funding source in accordance with corporate, contractual and regulatory standards. Coordinates and evaluates plan implementation.
Coordinates the development, review and revision of programs and behavior intervention plans. Provides, monitors and assures that comprehensive support services and oversight are provided on an on-going basis.
Schedules and/or provides opportunities for quality training, recreation and leisure activities with consideration for choice, preference and agae-appropriateness.
Maintains current and complete Mentor and staff reporting records in accordance with company and regulatory standards. Assists with all legal, licensing, regulatory and certification activities as needed.
Assists with pre-service and in-service training to Mentors. May assist in staff training, planning and development.
Maintains current and complete client records and reports in accordance with corporate and regulatory standards.
May secure individual Mentor contracts and complete annual contract reviews for Mentors. May also provide timely information as to any changes in client status and status of Mentor homes to management.
Monitors and assists in maintenance of documentation in the Mentor home including reviewing goal progress, daily progress notes, financial records, medication records, and fire/safety drills.
Conduct home visits as required/needed.
Participates in on-call rotation.
Performs other duties as required.
Key Requirements:
Bachelor's degree in human services or related field or equivalent in education and experience
Minimum one year of experience working with individuals with intellectual disabilities
Excellent oral and written communication skills
Proficiency in Microsoft Office products (Word, Excel, Outlook)
Ability to travel within area of responsibility
Reliable transportation for on the job use
Valid driver's license and good driving record
Successful background check clearance that meets company and regulatory requirements
