    As a Registered Nurse Behavioral Health, you will contribute to our success in the following ways:

    • Assess and identify the physical, psychosocial, and environmental needs of behavioral health care patients and families.

    • Take primary responsibility for your caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.

    • Incorporate data on physical, psychological, social, and spiritual factors into the patient's care plan.

    • Make initial evaluations and perform re-evaluations of nursing needs during each visit, ensuring follow-up visits as required.

    • Communicat e significant findings, issues, or changes in health condition to the appropriate stakeholders.

    • Revise the care plan based on identified patient care issues and notify the necessary team members.

    • Provide counseling , instruction, and support to patients, facilities, and families in following the Interdisciplinary Plan of Care.

    • Educate staff in alternate care settings on behavioral health philosophy and pain and symptom management.

    • Supervise and document the delivery of care by behavioral health aides and LPNs according to agency policy.

    • Report any unusual or potentially problematic nursing facility issues to supervisors.

    • Offer appropriate support during times of bereavement and conduct bereavement assessments.

    Requirements:

    • Current, unrestricted state RN license

    • Minimum of 1 year of experience in a clinical care setting as a nurse

    • Willingness to travel within the branch/office coverage area (up to 50%)

    • Reliable vehicle, valid driver's license, and current auto insurance in accordance with state laws

    • Preferred : Associates or Bachelors Degree in Nursing

    • Preferred : Previous Home Health or Hospice Experience

    • Preferred : Previous experience with Homecare HomeBase
