This position is a combined care coordinator/administrative assistant role designed to support one ACO-wide care management team such as the Transitions of Care team or the Pediatric Intervention Team while also providing administrative support to the BeHealthy Medical Director (BHMD). Adheres to system and department compliance policies, and any and all applicable laws and regulations. Performs other duties as assigned. Job Requirements: Expansion of the summary description. View ADA Requirements (http://eworkplace/portal/site/compensation/menuitem.a1af5bb58266f8e5a486a4862182a1a0/) 1) Support one ACO-wide care management team: coordinate and participate in daily team huddles;accept/coordinate referrals to team;communicate as needed with referral sources; conduct direct outreach to patients as needed to coordinate team interventions;set up team meetings; assist in tracking and coordinating team member activities; share data as needed with HNE analysts for return on investment analysis. 2) Generate reports from SMC List Management: Create monthly reports for 4 or more teams on team member activities, caseloads and patient facing activities for each care managed patient; work with SMC on any issues with report generation; share reports monthly with BeHealthy medical directors, teams and team leadership. 3) Serve as super user for SMC List Management: Troubleshoot problems;train new users. 4) Other reports: generate requested reports as needed. 5) Provide administrative support to the BeHealthy Medical Director (BHMD): meet with BHMD weekly to plan agenda for upcoming clinical meetings including Clinical Leadership, All Care Team, individual care teams (8), Direct Care Committee, Utilization Management Committee, and Patient and Family Advocacy Committee; schedule meetings, send reminders and distribute agendas; attend assigned meetings, take minutes, distribute minutes; maintain up-to-date staff roster for each team. Qualifications Unless otherwise required by certification, licensure, or registration, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the minimally required education and/or experience listed in the Qualifications section below. High School/GED Preferred Education: Associates Degree Minimally Required Experience: If none required select "none required ". 1-2 years office or administrative experience Skills/Competencies: Computer skills including proficiency with Microsoft Office, particularly Word, Excel and PowerPoint; ability to communicate well in writing and verbally.
