    POSITION SUMMARY

    ProMedica Toledo hospital is seeking a highly motivated Antimicrobial Stewardship and Infectious Diseases Pharmacist to join an established Antimicrobial Stewardship Team. The Antimicrobial Stewardship and Infectious Diseases Pharmacists works collaboratively with members of the ProMedica and University of Toledo Infectious Diseases teams through daily stewardship activities and key responsibilities listed below.

    ACCOUNTABILITIES

    Assists and supports the development, implementation and management of clinical pharmacy services. Provides clinical consultation and clarification to emergency department practitioners; recommends evidence-based medication therapy regimens and monitoring plans; suggests appropriate cost-effective therapeutic alternatives to medical staff as needed. Supports the development and implementation of treatment guidelines, policies, protocols, formulary reviews and critical pathways as approved by appropriate committees. Monitors medication therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory interactions, adverse drug reactions, appropriateness of drug and dose and therapeutic duplications. Provides accurate, adequate and timely drug information to professional staff. Works collaboratively with clinical pharmacy specialists, clinical staff pharmacists, emergency department physicians, advanced practice providers, medical residents, nurses and case management. Supervises and instructs pharmacy students and pharmacy residents, if applicable. Participates in antimicrobial stewardship program. Prepares and presents medication use evaluations, adverse drug reaction reports, medication errors, and pharmacy interventions summary to the P&T committee. Supports and participates in the pharmacy's performance improvement activities including customer service and patient safety initiatives and documents departmental compliance. Establishes productive, collaborative relationships with staff and community members. Performs duties of Staff Pharmacist as needed. Performs other duties as directed. Performs all duties and responsibilities in an ethical and responsible manner. Supports the department in providing a vision for its direction and embraces the change process to achieve the vision. Fosters the culture of patient safety by using the error prevention techniques. Coaches and facilitates the growth and development of employees within the department. Communicates effectively orally and in writing within department as well as to all external customers. Strives for continuous improvement by focusing on key department systems. Participates in creating an environment for success through team participation.

    Education: Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy. Skills: PGY1 and/or five years of experience as a pharmacist in acute care hospital required. Toledo and Flower Hospitals require residency as they precept residents at those locations. Years of Experience: 5 years of experience in Antimicrobial Stewardship or Infectious Diseases. License: Pharmacist licensure in current state of practice. Certification: As applicable, must complete GCP and CITI training upon hiring and maintain certification.

    PREFERRED QUALIFICATIONS

    Education: Doctor of Pharmacy Degree, Completion of PGY2 Infectious Diseases Pharmacy Residency is preferred Certification: Board Certification (BCIDP), BLS - Basic Life Support

    WORKING CONDITIONS

    Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; frequent standing; occasional squatting, kneeling, walking, and stairs. Must be able to move about and between workstations.
