    HOME HEALTH NURSE (RN)

    Shift: Mon to Fri, 8 am to 430 pm, rotational on-call telephone support

    Status: Full time

    Location: Saint Francis LIFE Riverfront, Wilmington, DE

    Incentives: $ 2,500 Sign on Bonus

    Position Purpose:

    The Registered Nurse (RN) is responsible for providing oversight and management to the provision of service delivery to participants. Provide skilled nursing care, prescribe treatments, and develop appropriate plans of care to participants according to professional nursing standards and primary care orders.

    Position Details:

    This position qualifies for a $2500 sign on bonus with a 2-year commitment. The 3 installments are paid out over the two-year commitment period.

    One weekend per month, will work home care assignments on day shift and participate in on-call rotation. On-call will be remote via telephone with no on-site reporting required.

    This position will primarily consist of home care visits as well as occasional work at the facility.

    What you will do:

    • Provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders Complete participant nutritional assessments and reassessments as needed.

    • Participate as a member of the interdisciplinary team (IDT), including completion of initial medical history, physical exam, and functional nursing assessments of each new participant and semi-annual, annual, and unscheduled assessments.

    • Participate in daily program activities and provides comprehensive plan of care to meet participant needs and promote optimum health, independence and self-care.

    • Facilitate integration of new participants into the PACE program health care delivery system.

    • Demonstrates nursing care to and instructs participants and families in treatments and other self-care interventions.

    • Communicate, counsels and guides participants and families regarding service needs and concerns as well as providing education on health-related topics.

    Minimum Qualifications:

    • Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Pennsylvania.

    • BSN preferred. Active CPR Certification required.

    • Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.

    • Valid driver's license in good standing and proof of insurance required. Use of personal vehicle required for each assigned shift.

    • Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services.

    • Demonstrates superior written, verbal communication and presentation skills. Able to effectively communicate with those representing diverse perspectives.

    • Proficiency with Microsoft Office - Word, Excel, Outlook. Ability to use other software as required to perform the essential functions of the job.

    • Ability to change and be flexible with work priorities.

    • Position requires frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
