    Who we are:

    For more than 80 years, the Centers for Independence(MCFI) has strengthened communities by helping people advance their total health. MCFI's Pediatric Skilled Nursing program provides daycare services Monday-Friday to children aged birth-21 years old that require skilled nursing care due to their complex medical needs. Our staff of RNs, CNAs, and childcare teachers utilize a team based approach to provide direct medical, social, and educational care to children within our 4 childcare classrooms.

    Shift Description:

    • 4x10hr

    • 3x12hr and 1x4hr

    • No nights, weekends or holidays

    • Flexible scheduling with start times ranging from 0600-0800 and end times ranging from 1630-1830.

    Position Summary:

    The Registered Nurse is accountable for providing direct and indirect patient care to children with special health care needs and their families in accordance with the nursing process and established policies and procedures, the nurse practice act, local, state, federal and Joint Commission standards.

    Work Environment and Physical Requirements

    The work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job:

    • Must frequently be able to move, transport, position, push/pull and/ or lift objects or equipment weighting at least 50 pounds a distance of 450 feet.

    • Ability to successfully deal with change and stressful situations.

    • This position is responsible for the supervision and safety of clients in a multi discipline care setting. As such, this position must be able to observe client activities, detect areas of danger, and distinguish when clients who are non-communicative are in need of personal cares, and converse clearly with clients and staff on a frequent basis.

    • This position must frequently and effectively communicate complex ideas and instructions to other staff.

    • Must be able to safely push a person in a wheelchair on a level floor or on a wheelchair ramp in various weather conditions. Must be able to transfer a person from a wheelchair to the toilet, chair, or van seat/car with use of a gait belt and/or manual or power Hoyer lift.

    • PSN operates from 6am to 6pm 5 days a week and requires flexibility to cover the needs of the business as it evolves and changes.

    • This position requires the employee to wear designated PPE to include: N95 mask, gowns, gloves, and designated face shield/eye wear for the entirety of their shift to ensure compliance with Infection Control standards.

    Essential Job Functions :

    • Assesses, plans, implements and evaluates nursing care based upon the patient's physical, psychological, emotional and mental wellbeing in accordance to the Plan of Treatment, the needs of the patient, and the needs of the family.

    • Carries out clinical skills with accuracy and competency to meet the standards of care established by Pediatric Skilled Nursing.

    • On an ongoing basis makes patient assessments, revises the Plan of Treatment as deemed necessary in collaboration with the physician.

    • Collaborate with nursing supervisors to update the MAR (medication administration record) , review discharge orders, and physician orders as needed.

    • Revises the Kardex on an ongoing basis.

    • Actively engages in teaching new skills and reinforcing learned skills including but not limited to, general wellness techniques, safety, diet and nutrition, new procedures and equipment, medications, and emergency procedures.

    • Uses effective communication skills with patients and families.

    • Communicates with physicians on an ongoing basis.

    • Oversees all paraprofessional staff including students, volunteers, CNAs and provides directives as appropriate.

    • Uses effective telephone listening and communication skills to assure safe, quality care according to the standards of care of Pediatric Skilled Nursing.

    • Uses critical thinking and problem solving skills to overcome obstacles that may interfere with the quality of nursing care delivered.

    • Completes all documentation of patient care within a timely manner in accordance with Pediatric Skilled Nursing policies.

    • Responsible for participating in the orientation of new nurses to the PSN.

    • Cognizant of case management needs of patients/families and works with the clinical nurse supervisor to establish and implement a plan with established outcomes and time lines.

    • Maintains patient/family confidentiality at all times

    • Participates actively in care conferences and home visits as needed.

    • Works in partnership with leadership regarding quality improvement practices to develop appropriate measures in support of program outcomes.

    • Participates in department quality improvement planning activities.

    • Collaborates with RN Supervisors to reconcile medications.

    • Collaborates with RN Supervisors and Child Care Supervisor to create assignments and transportation nurses for following day.

    Required Technical Skills, Experience, Education, and Credentials :

    • Graduate of an accredited School of Nursing

    • Current WI Nursing License, without limitations that are incompatible with duties, responsibilities, supervision or practice setting

    • Must be CPR certified at the start of hire and must maintain active CPR certification through periodic renewal as dictated by the certification.

    • Experience with pediatric special needs clients is preferred.
