    The Population Health Physician is responsible for providing physician leadership to support Somatus' Population Health care teams in each patient population market. With a focus on achieving improved quality outcomes for patients, the Population Health Physician provides clinical leadership and accountability, working collaboratively with data/analytics and operations teams, to drive improved quality and utilization outcomes across a multi-state market geography. In addition, the Population Health Physician may support provider engagement and market level client relationships with a focus on clinical collaboration to advance patient care in support of Somatus' mission.

    Responsibilities

    • Provides physician leadership to field care teams providing care management services for patients and reviews comprehensive patient care plans and identifies opportunities for care improvement across the spectrum of patient's total health.

    • Serve as collaborative physician to provide oversight to nurse practitioner(s) performing risk adjustment visits in relevant states.

    • Provides physician leadership presence to market field teams and clinical oversight to drive key quality and utilization performance outcomes.

    • As needed, conducts case reviews to help identify opportunities for local and market field teams to improve quality of care.

    • Applies evidence-based clinical guidelines in assessing appropriateness of utilization to identify opportunities for improved efficiency of care.

    • Supports market operations teams with development and implementation of Somatus population health clinical programs, including clinical quality oversight of care team execution, in order to achieve improvement in clinical, utilization, and total cost of care outcomes.

    • Partners with SVP Medical Management to improve market-specific clinical outcomes (quality metrics) and utilization outcomes (admission rates, ED utilization rates, and re-admission rates).

    • Provides physician representation alongside Market Vice Presidents with market health plan clients, with a focus on enhancing collaboration to advance clinical care.

    • Supports market provider engagement teams, including building relationships with local providers as well as providing clinical oversight to Somatus care teams supporting local primary care and nephrology practice partnerships.

    • Conducts peer-to-peer discussions where needed with local treating nephrologists, PCPs, and hospitals as needed, focused on clinical collaboration for patient care.

    • Provides ongoing education and guidance in leading care team members, with a focus on teammate development and growth.

    • Participates in quality improvement work across the organization.

    • Maintains strong knowledge of medical and social issues, CKD & ESKD research related to prevention, care and treatment advancements.

    • Understands and supports patient stratification, continuous evaluation, and re-stratification of members for appropriate resource allocation.

    • Ability to remain flexible to accommodate and support business needs

    Qualifications

    • Graduate of an accredited medical school. M.D./D.O. degree is required.

    • Completion of approved training in internal medicine, family medicine, primary care, emergency medicine, or nephrology and maintains active board certification.

    • At least 2-3 years of post graduate experience caring for complex adult populations.

    • Possess a valid, unrestricted license to practice medicine in relevant states and willing to pursue additional licensure as required for job duties.

    • Demonstrated experience in population health or utilization management.

    • Excellent interpersonal, verbal, and written communication skills.Consistently completes continuing education activities relevant to practice area and needed to maintain licensure.
