    Position Overview:

    Manages clinical operations and ensures department success in meeting the organizational goals and initiatives of patient and team safety, affordability, quality, patient satisfaction and access. Plans, organizes, and directs the development, implementation, and improvement of patient care services in accordance with current standards and regulations. Models and promotes Lean principles in establishing standardized best practice across departments. Accountable for all clinical and business developments effecting assigned departments and specialties. Provides oversight and guidance to Supervisors and staff to maximize efficiency and develop processes that improve workflow. Develops and maintains a solid partnership with internal and external peers, leaders, and/or local/state/federal agencies to facilitate and collaborate on initiatives or issues resolution.

    EDUCATION:

    Graduate of an accredited school of nursing.

    Masters in healthcare preferred

    CERTIFICATION & LICENSURE:

    RN-Registered Nurse of California.

    Current California RN license.

    Current BCLS/CPR certification.

    California Drivers License

    Automobile Insurance

    TYPICAL EXPERIENCE:

    8 years recent relevant experience.

    OTHER

    Valid California driver's license; current auto insurance.

    SKILLS AND KNOWLEDGE:

    Knowledge and expertise in clinical decision-making for the patient populations assigned, to meet the needs of the patients and to contribute to Quality Management review and evaluation.

    Solid understanding of clinical operations and workflows and how discipline specific process/workflow impacts other department operations.

    Demonstrated leadership skills, including team building, and coaching/mentoring with the ability to motivate and engage others.

    Working knowledge and thorough understanding of state/federal/local and/or accreditation standards and requirements as it relations to operations and organization, with the ability interpret and implement applicable standards and requirements.

    Demonstrates ability to develop and manage a budget.

    Demonstrates tactical and analytical thinking and planning skills, and is able to develop plan to accomplish objectives, identify obstacles and resolve operational issues.

    Ability to build and establish effective working partnership with employees, peers and/or leaders to achieve business objectives.

    Excellent verbal and written communication, and interpersonal skills.

    Ability to handle difficult circumstances and make sound business decisions with little direction.

    SPECIFICS FOR ADVANCED ILLNESS MANAGEMENT ADMINISTRATOR ROLE

    . Minimum of 5 years relevant and progressive management experience to include at least 3 years' experience in management of home health or hospice. Palliative Care experience also preferred. Other home care-based experience may be considered.

    • Minimum of 5 years of direct patient care of patients with advanced chronic illness, palliative care, or home health or hospice care and familiar with care management principles, psycho-social, spiritual and cultural aspects of care.

    • Ability to demonstrate communication, presentation, leadership, analytical and problem-solving skills.

    • Demonstrated ability to lead and manage change through the use of interpersonal skills such as negotiating, collaborating and influencing.

    • Demonstrated ability to achieve results on both a strategic and operational level in collaboration with diverse stakeholders.

    • Demonstrated expertise in program management.

    • Ability to be a strong and consistent mentor to staff and to use disciplinary process appropriately

    • Strong interpersonal skills to navigate sensitive issues and conflicts with or among others, and respectively direct and reinforce staff efforts in the management of populations with complex needs within an integrated care delivery framework.

    • Excellent understanding of home health, hospice and palliative care services, Title XXII, Medicare Conditions of Participation, home care, quality management and performance improvement (PI) outcomes/benchmarking, infection control; good understanding of reimbursement patterns for Medicare and other payers; good working understanding of fiscal responsibilities, budget and financial reports. Knowledge of available healthcare and community resources appropriate for population served. Knowledge of home-based clinical practice and risk management as applicable to population and setting.

    Self-starter with a high degree of initiative, motivation, flexibility, energy and creativity. Ability to form harmonious working relationships with internal and external customers, and able to facilitate positive and productive working relationships among co-workers and subordinates. Exhibits excellent oral and written communication skills; possesses competence in computer work processing skills, understanding of the principles of adult education and delivery of training; collaborative and problem-solving skills. Access to a reliable automobile for travel in assigned coverage area and for other related work assignments.

    Pay Range is $73.25 to $117.20 / hour

    The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
