Primary Care New Patient Intake Nurse - Partial Remote Baystate Health, western Massachusetts's premier healthcare provider, and home to Baystate Medical Practices is looking for an experienced Registered Nurse to join our P RIMARY CARE TEAM RESPONSIBILITIES + Completes a comprehensive chart assessment to identify gaps in information that would prevent a seamless transition + Reviews patient history to ensure appropriate booking based on patient complexity + Compiles patient medical, social, and family histories for inclusion in the medical record for new patients + Interact with other departments, physicians, and staff to assure delivery of quality care to patients prior to the initial visit . + Work with patients to understand health care maintenance, required pre visit testing, medication reconciliation and appropriate avenues of office communication. + Help develop and reassess the onboarding process for new patients + Develop and update appropriate written patient education materials Work Experience: + 3-5 years experience as a Registered Nurse- must have MA license and CPR + Primary care, medical practice or intake experience strongly preferred + Ability to work autonomously + Exceptional communication and computer skills POSITION HIGHLIGHTS + PROFESSIONAL DEVELOPMENT AND SUPPORT.
