    Summary

    Responsible for coordinating a comprehensive, evidence based Joint Commission Disease Specific Certified Stroke program, New York State Department of Health Certified Stroke Center and compliant with American Heart Association/American Stroke Association Gold standards for Stroke Get With the Guidelines. This position coordinates the development of the program including leading the multidisciplinary teams, collaborating with the Department of Neurology, Neurosurgery, and Interventional Radiology for the development and implementation of protocols, policies, order sets, and educational materials.

    Responsibilities

    1. Effectively plans, prepares, and coordinates functions for stroke program regulatory agency surveys.

    2. Provides leadership and education for the stroke multidisciplinary team to ensure compliance with stroke protocols, evidence-based standards, and regulatory agency requirements.

    3. Develops and maintains a stroke database for Joint Commission/ CMS core measures, meaningful use performance measures, New York State Department of Health stroke center designation and American Heart Association / American Stroke Association Get With the Guidelines for Stroke.

    4. Monitors stroke care from emergency department admission through treatment and stabilization to discharge or transfer to a higher level of care.

    5. Applies clinical knowledge to accurately collect a wide variety of stroke clinical performance process and outcome measures from medical records within the designated time frames.

    6. Provides education and collaborates with medical and hospital staff to ensure compliance with all stroke performance measures.

    7. Submits stroke process and outcome performance measures to Joint Commission/ CMS, New York State Department of Health, American Heart Association / American Stroke Association.

    8. Develops and maintains the Stroke Program Performance Improvement Plan through analyzing aggregate reports of stroke clinical performance measures and identifying external comparisons for performance measures.

    9. Collaborates with the multidisciplinary stroke team and the Department of Neurology and Neurosurgery to coordinate the implementation of practice changes as needed.

    10. Prepares and participates in the tri-annual Quality performance improvement council report from the Department of Neurology and Neurosurgery.

    11. Collaborates with the multidisciplinary stroke care team and Department of Neurology and Neurosurgery to develop protocols, policies, and order sets to support evidence-based stroke care.

    12. Facilitates provision of educational opportunities for staff, physicians, patients, external providers of care and the community & coordinates community outreach programs for the stroke program.

    Other information

    Education and Experience Requirements:

    • Bachelor of Science in Nursing.

    • Minimum 3 years of acute care hospital experience and basic computer skills.

    • Preferred: Prior experience in performance improvement activities.

    License, Registration, or Certification Requirements:

    • Current New York State Registered Nurse license required.

    • Ability to communicate and interact with all levels of professionals, including physicians, nursing personnel and managers in a non-confrontational manner
